New Patient Registration

We are using a new online service called Register with a GP surgery that makes it easy to register with us.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

Please note

We request each person to provide the following information to the practice staff in order to facilitate the registration with the Practice: Registration can still take place if documents are not available but processing and verification may take longer.

  • Proof of ID
    Passport, Driver’s Licence or Birth Certificate.
  • Proof of Address
    Copy of utility bill or bank statement within the last month from the time you are registering or a current Tenancy Agreement.
  • NHS Number
    If you have previously registered with a GP, we will require your NHS Medical Card or your NHS Number. If you cannot recall it, then we will require contact details of previous GP. However, your previous GP may not provide details due to confidentiality and formal requests may take time.

Please be aware that if you are a Visitor to the UK, we may be able to register you under the NHS but further services and referrals will incur cost. If you are a EEC Resident, you may utilise your European Health Insurance Card.

A health questionnaire is required for registration. This will provide the clinicians an insight into your health status.

Upload copy/scan of child immunisation or immunisation page from red book.

You are required to bring your child’s Red Book to your Nurse appointment. If your child does not possess one, please inform the Nurse and one will be issued to your child.

Once these requirements are completed, the Practice Nurse will perform a full health check.

You do not need your NHS Number to register as a patient.  However, in order for us to achieve a smooth and error free registration your NHS Number is an integral part of your care.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.

Temporary Patients

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
